David Davis: To ask the Secretary of State for the Home Department how many (a) asylum seekers, (b) tourists and (c) immigrants entering the UK were tested for (i) tuberculosis, (ii) HIV and (iii) hepatitis in each year since 1997; and what proportion tested positive in each case. [217742] Mr. Charles Clarke: Referral statistics are not collected routinely for all ports of entry. However we have obtained the statistics shown in the table for tuberculosis checks at Heathrow and Gatwick from the bodies which carry out the checks there. Mr. Luff: To ask the Secretary of State for Health what proportion of the population of England he estimates to be undiagnosed for (a) tuberculosis and (b) hepatitis C. [200103] Miss Melanie Johnson: It is not possible to generate reliable estimates on numbers of undiagnosed active tuberculosis cases in England. Most undiagnosed cases present to the health service at some point during their illness. Delays to diagnosis will result in either recovery, eventual presentation to health services with advanced disease or death. The number of active tuberculosis cases who recover is unknown and numbers of cases who present with advanced disease are not routinely collected. The number of cases of tuberculosis diagnosed on post- mortem is collected by the Health Protection Agency's (HPA) enhanced tuberculosis surveillance system. This data represents the number of tuberculosis cases per annum who are found to have tuberculosis at post mortem. Tuberculosis is however not necessarily the cause of death or a contributory factor to death. In a large proportion of these cases, tuberculosis is likely to be an incidental finding. The number of cumulative hepatitis C laboratory diagnoses reported, 38,000, to the HPA is lower than the estimate for the overall population prevalence. This suggests that the majority of hepatitis C infections have not been diagnosed. The estimate of the number of individuals infected with hepatitis C is 250,000, but the figure for those who have gone on to develop chronic hepatitis C infection in England is 200,000, as in some cases the infection clears spontaneously.

[144006] Miss Melanie Johnson: [holding answer 15 December 2003]: The Cabinet Office is currently co-ordinating work between relevant Government Departments, including the Department of Health, to review imported infections and immigration. The review aims to establish the facts about the impact of immigration on public health and national health service expenditure, consider all relevant issues including health screening and propose solutions should action be required. It is on-going and no decisions have been taken yet. Asylum seekers are offered health assessments and screening for tuberculosis (TB) at the Home Office induction centre in East Kent (and will be offered them at other induction centres as they are rolled out). Testing for HIV is offered as part of the health assessment on request or where medical history indicates they have been at risk. The aims of the health assessment are to identify the immediate healthcare needs of the asylum seeker and to protect public health. Imposing mandatory health screening on asylum seekers would breach their human rights and would be contrary to the 1951 Refugee Convention. In relation to other migrants, long-standing policy is that any person subject to immigration control who either mentions health or medical treatment as a reason for coming to the United Kingdom, or appears unwell; or is seeking leave to enter the UK for six months or more and is at high risk of having been exposed to TB should be referred by the immigration officer to a medical inspector. These arrangements are governed by Schedule 2 of the Immigration Act 1971 and the Statement of Changes in Immigration Rules (HC 395).

Mr. Flynn: To ask the Secretary of State for Health what assessment he has made of the likely change over the next 10 years in the incidence of cases of (a) MRSA, (b) TB and (c) Hepatitis C; and what action is he taking to deal with such changes. [105424] Yvette Cooper: [holding answer 20 January 2000]: MRSA is the antibiotic resistant form of Staphylococcus aureus, a bacterium which is carried harmlessly on the skin by at least a third of the population. About 80 per cent. of people who acquire MRSA similarly carry it harmlessly on the skin and do not suffer from any infection. Action is in hand to improve the surveillance of MRSA and also the prevention and control of hospital acquired infection, including MRSA. The extent to which this work will impact on the cases of incidence over the next 10 years is unpredictable. Official notifications of tuberculosis gradually fell to an all time low of 5,086 in England and Wales in 1987 from about 50,000 cases a year in the 1950s. Since the early 1990s there have been overall small year on year increases in total notified cases which are continuing. Provisional notifications for 1998 were just over 6,000. While some of this may be due to better ascertainment of cases, it has occurred against the backdrop of a worldwide resurgence of this disease that will, inevitably, continue to have some impact on tuberculosis in the United Kingdom through travel and immigration. We are working to minimise the effect of worldwide developments and have taken steps to strengthen the surveillance, prevention and control of tuberculosis, including drug resistant tuberculosis. The incidence of hepatitis C infection is not known as the virus is usually acquired without symptoms. Currently, the most frequent route of transmission of hepatitis C is through sharing of blood contaminated equipment by injecting drug misusers. Preliminary evidence suggests that the prevalence of hepatitis C infection in newer injectors may be lower than in the past. We are continuing to promote harm minimisation measures to try to accelerate any downward trend. However, there is likely to be an increase in the diagnosis of hepatitis C in the next 10 years as individuals who have carried the virus for some time are identified through wider testing of groups who have been at risk. Improved treatments are becoming available to help prevent progression to serious liver disease.
