My Lords, we are now introducing a system of pre-entry screening. We recognise the contribution that latent TB makes to the overall TB disease burden and that is why we have commissioned NICE to produce a clinical guideline on this. In the countries where TB presents the most significant risk, we shall in future insist that people are screened before they enter the United Kingdom.

I thank my noble friend for that encouraging reply, but is he aware that the chaotic lifestyle of homeless people with very poor immune systems means that they are extremely difficult to diagnose and to treat for the six-month period for which they need antibiotics? Are Her Majesty’s Government working with other organisations to help these homeless people?

My noble friend is absolutely right: this is a particularly difficult group of people in that they are hard to reach. There is a high incidence of TB among the homeless in London and a service of the kind to which I have referred appears to be cost-effective in reaching those people. On my noble friend’s second question, we are engaging with the Mayor of London’s office to see how it can become involved in helping to deliver cost-effective services to this group of people.

My Lords, from 1 April, the National Health Service in London will fund continued provision of the Find and Treat outreach service to detect TB among the homeless, including rough sleepers, and to help to ensure treatment completion. There are also initiatives for TB testing among the homeless in cities such as Liverpool and Leicester. The National Institute for Health and Clinical Excellence is developing guidance on tuberculosis among hard-to-reach groups, including rough sleepers.

Increasing awareness of tuberculosis (TB) among groups at risk of infection and healthcare professionals and providing accessible services are key factors in improving early diagnosis and treatment.
The department is funding TB Alert (a United Kingdom charity) to increase awareness of TB among groups most vulnerable to TB and among primary care health professionals. This is supported by the Health Protection Agency.
In addition, the department is supporting National Health Service TB services in London by piloting a Find and Treat service to identify cases of TB among people who are homeless or have other complex social needs, and ensure they have prompt support and complete treatment, to help prevent drug-resistant TB.

The noble Baroness is correct. She is an expert on these matters. Seventy per cent of TB cases in England occur in people who were born abroad but show no sign of illness after living here for many years. We have, as yet, no test that can predict who will develop TB in later life. We know that there are no short-term solutions in tackling TB. The long-term action will take time to take effect, but the strategy in this country is to detect it early. That means that the generation of doctors who thought that TB was a problem that we no longer needed to address in his country need to be trained to recognise TB, diagnose it and make sure that people go through the treatment, which, I am happy to say, is successful, to make sure that we deal with this in this country.

In response to the second question: indeed they are. Our data show that around 70 per cent of the TB cases in England are among people born abroad, but who show no signs of the illness until they have lived here for many years. At the moment, as the noble Baroness will be aware, we have no test to predict who will develop TB in later life.

My Lords, we know that the problem of TB is limited to particular localities these days. In these areas, PCTs are able to set their own targets and standards as part of a local delivery plan agreed with their strategic health authority. In respect of standards of treatment, there are clear recommended guidelines from the National Institute for Clinical Excellence.

I am not sure that having had TB is a declaration of interest, but the noble Baroness’s experience has helped us with our discussions. As she knows, screening new entrants for TB has been a long-standing government policy, as indeed has pre-entry screening in some of the countries. However, 80 per cent of people born outside the UK who develop infectious TB do so only after living in the UK for two years.
In my previous Answer, I referred to the fact that the HPA would be reviewing the effectiveness of port screening and there is a cross-governmental review of pre-entry screening to consider this in relation to TB management and how it affects the migrant population in general. The result of that will be led by the Department of Health and the Home Office. However, experts maintain that early detection and completion of treatment in tackling TB is a priority at local level and that is why our awareness-raising campaign is so important.

The Chief Medical Officer’s 2004 TB action plan makes raising awareness of TB one of its top priorities. The Government funded TB Alert to raise awareness among communities at particular risk and healthcare professionals, and also Find & Treat to work with vulnerable groups, such as the homeless, to increase uptake of TB screening.

My Lords, I thank the noble Baroness for that Answer. Does she agree that one in three of the world’s population is infected with latent TB? Why are there not sufficient funds to do a blood test on people with possible latent TB, a test which costs only £30 whereas treatment of drug-resistant TB costs £60,000 per patient?

My Lords, the noble Baroness is absolutely right about the problems with drug-resistant TB. She will also be aware that there have been a very small number of cases. In the UK since 1993 we have identified eight cases, three of which needed treatment—the other five patients died—and all the cases have been confined to patients with certain types of lifestyles, involving people who are homeless or dependent on drugs. We attempt to identify those people as early as possible and ensure that they get treatment and follow through the course of treatment.

My Lords, the Government are committed to tackling TB, and in 2004 the Chief Medical Officer for England published an action plan. Our key strategy is early detection and completion of treatment. To implement this, we launched an information pack designed to help NHS commissioners secure best services to meet local needs, and we are funding an awareness-raising programme for the public and professionals to help earlier identification of TB.

My Lords, I agree with the noble Baroness. Individuals immigrating from abroad who have a high susceptibility to tuberculosis are currently screened. There are two screening mechanisms. First, there is a screening pilot, which we started in 2005, in which all individuals staying more than six months in the UK are screened at the time of acquiring a visa. If they happen to have active disease, they will be treated completely at their own cost until they are free of disease and before they obtain a visa. These countries happen to be Bangladesh, Sudan, Tanzania, Thailand, Ghana, Kenya and Pakistan. If individuals are coming from other countries in which the prevalence is greater than 40 per 100,000, they will be screened at the port of entry.

My Lords, data indicate 39 TB incidents in schools in London to date this year. Since data collection began in 2004, there have been 148 TB incidents in London schools, which include nurseries but exclude higher education. Incidents are defined as occurrences where potential transmission to non-household contacts is identified as a public health risk warranting wider investigation. Outbreaks are a specific sub-set of incidents. It is not possible to provide data on outbreaks alone.

Perhaps we cannot catch up with them when they disappear into the system; I gather a lot of immigrants disappear and we do not know where they are.

My Lords, it is interesting that the noble Baroness said that she had had TB. A third of the world’s population has the TB bacteria. The total number of cases of TB in the UK in 2006 was 8,479; that is the latest figure we have. Of that, 72 per cent of new TB cases were non-UK born. However, the vast majority of these cases only develop active disease after they have been in the UK for at least two years, and often much longer than that.
The noble Baroness asked about screening. Of course, we have a long-standing policy, as the noble Baroness will know. Anyone who appears unwell, mentions health problems or medical treatment and is seeking entry for more than six months will be screened. As the noble Baroness will know, there is a list of high-prevalence countries for which there is screening.

My Lords, the UK provides support for TB control through its funding to the Global Fund to Fight AIDS, Tuberculosis and Malaria; through bilateral country programmes; through organisations such as the World Health Organisation; and through new research evidence and drugs. The Prime Minister, then Chancellor, launched the Global Plan to Stop TB with former President Obasanjo, of Nigeria, and Bill Gates, co-chair of the Gates Foundation, in 2006.

My Lords, we currently have a pilot project with the seven countries in which TB is most prevalent, in which we test people before they receive their visas to come to the UK. That scheme is now being evaluated, and we will probably roll it out to more countries such as China and India on the basis of that evaluation.

My Lords, the Department of Health recommends that TB services follow the National Institute for Health and Clinical Excellence guideline on the prevention, treatment and control of tuberculosis. It contains specific guidance on the follow-up of new entrants. The advice is also emphasised in the toolkit recently published by the department to help NHS trusts plan, procure and deliver TB services relevant to the local demography and incidence of TB.

My Lords, I can assure the noble Baroness that we are taking every step to ensure that we have a robust response. The first step is to screen those who are coming into this country and, if they are identified as carriers of tuberculosis, to deal with it before they come here. The second step is to ensure that those who come to this country are progressively screened so that we can identify the disease early and deal with it more robustly.

My Lords, currently, we screen for TB those arriving from countries with a high rate of TB who want to remain for longer than six months. We also screen abroad applicants for entry clearance in six countries that are high-risk for TB. We will shortly be making an announcement on the next phase of this overseas screening programme, which will extend the scheme to other high-risk countries.
