Chlamydia screening data from genito-urinary medicine (GUM) clinics is part of a clinic aggregate return (KC60) and is not currently available by primary care trust (PCT) of the patients or age of patient.The Department and the Health Protection Agency are implementing a new sexually transmitted infection data set for GUM, which will replace the KC60. This will provide data on numbers tested for chlamydia (under 25) in GUM services by PCT of residence. We expect full national roll-out later this year.The National Chlamydia Screening Programme was set up to target asymptomatic individuals under the age of 25, who were not usually presenting for chlamydia testing. Information on the number of people screened is collected by PCT of residence and data is assigned to each individual PCT.For the first year of the vital signs indicator (2007-08), relating to chlamydia, the emphasis was on counting and increasing all community testing. Once testing is sufficiently embedded within the community, we will then use all community data along with GUM tests to monitor progress towards reducing chlamydia prevalence in each PCT.

Information on the costs to the national health service of Chlamydia testing in genitourinary medicine (GUM) clinics, general practitioner (GP) surgeries and pharmacies is not available centrally.In GUM, Chlamydia testing is routinely undertaken as part of a broader sexual health screen. In 2009-10, GUM clinics will attract the national tariff payment of £139 for each first attendance and £86 of £119 for any follow up attendance depending on the clinical complexity.For Chlamydia screening within the National Chlamydia Screening programme (NCSP), in an article "Modelling the healthcare costs of an opportunistic chlamydia screening programme" published in the Sexually Transmitted Infection Journal on 1 April 2004, average estimates costs based on the pilot screening model in general practices and contraceptive and sexual health services are as follows:"for those screened and not infected (the majority of those screened): £18-£25;""for those screened and infected (includes costs of treatment and initiation of partner notification): £34-£42; and""for partner treatment: £24-£30."A copy of the article has been placed in the Library.In a further article, "The health care and patient costs of an proactive Chlamydia screening programme: the Chlamydia Screening Studies (ClaSS) project", published in the British Medical Journal online, 17 January 2007 came up with similar costings. A copy of the publication has been placed in the Library.The NCSP is currently reviewing costing of Chlamydia screening in the community and developing appropriate tariffs, in collaboration with the contraceptive and integrated sexual health services tariff development group.Different variations of Chlamydia screening pathways are being costed: in Contraceptive and Integrated Sexual Health Services, in general practices, in pharmacies, mail outs and postal Chlamydia screening kit. This review will be completed later this year.

Data on the number of sexual health screens, which includes a test for Chlamydia, in genitourinary medicine (GUM) clinics between 2003 and 2007, the latest year for which figures are available, are shown in the following table.
In addition to sexual health screens in GUM clinics, the National Chlamydia Screening Programme (NCSP) provides screening for Chlamydia to asymptomatic people aged under 25 years of age in healthcare and non healthcare settings across England. The NCSP was launched in 2003. The following table shows the total number of Chlamydia tests done in general practice and pharmacies among 15-24 year-olds during the period 1 April 2003-31March 2008.

Data on sexual health screens are not available for 1997 as the sexual health screen codes have only been collected since 2003.The number of sexual health screens undertaken in genito-urinary medicine (GUM) clinics in England, by gender; for the years 2003 and 2007, (the latest date for which figures are available) are shown in the following table.

The strategy published last week addresses all those issues. I am pleased to see that the hon. Lady is concerned about health inequalities, because we want our health centres and our out-of-hours facilities to address them. Screening with specialist nurses, in particular, will take place so that we can look after members of BME communities in a special and different way.

The Department launched the ““Condom Essential Wear”” campaign in November 2006, to tackle sexually transmitted infections and the behaviours that contribute to their spread. This campaign particularly features chlamydia and gonorrhoea in its creative executions.The Department is also investigating how promotional activity at a national level can support the delivery of the National Chlamydia Screening programme.

Officials within the Department have discussed Chlamydia screening with strategic health authorities (SHAs) in both formal and informal settings on a number of occasions within the last six months. The following table shows the dates when officials met with SHAs and at most of these meetings Chlamydia was discussed. The content of the discussions focused on the performance of SHAs in relation to Chlamydia screening.
Performance on Chlamydia screening is one of the indicators used to inform primary care trusts rating through the Healthcare Commission Annual Health Check in 2007-08. Reducing Chlamydia prevalence has been identified as a national priority target for local delivery in the national planning guidance for the national health service in the 'NHS Operational Plans 2008-09 to 2010-11', which highlights the Government's continuing commitment to tackling this infection. A copy of the guidance is available in the Library. We will continue to work through performance management routes to support delivery of this target and increase screening volumes.

The sexual health national support team (SH-NST) was set up in February 2006 to support local areas to deliver against the 48 hour genito-urinary medicine access target. Since inception the national support team has undertaken 62 visits and provided associated ongoing support to local areas across England. The total staffing and visit costs of the team to date is £1.2 million.In 2008-09, the SH-NST will be shifting its focus to providing support on the Chlamydia screening indicator. It will work closely with the National Chlamydia Screening Programme (NCSP) to increase screening volumes, working jointly with strategic health authorities to identify those areas facing the greatest challenge and supporting and monitoring the NCSP to ensure rapid progress to target.

The National Chlamydia Screening programme (NCSP) published the Men too Strategy in November 2007 in response to the variation observed in the proportion of men screened in the local Chlamydia Screening programmes. All chlamydia screening offices (CSOs) have been made aware of the need to address the issue of equitable access to screening.Discussions on ways to increase uptake amongst men have been undertaken at a number of region-wide chlamydia screening co-ordinator networks. Work in relation to identifying interventions supporting male engagement and the success of each activity is underway through consultation with local Chlamydia Screening programmes. This feedback will be collated to inform a working group which has been convened to advise and steer the implementation of the ““Men too Strategy”” later in 2008.The working group includes representatives from the Men's Health Forum; a men's health promotion specialist from a primary care trust (PCT); a sexual health network director, who commissions and facilitates sexual health service provision; the chief executive of Brook, the young persons sexual health service; and a CSO coordinator.Work in relation to men's perceptions and experiences has been discussed by the working group but no formal study of this has commenced yet.Work on background information and guidance is in the early stages of development. These will be published in the form of a toolkit planned for launch in November 2008.To date, examples of local initiatives include the Greater Manchester strategy to use sport to engage male participation in screening. In partnership with both Oldham Athletic Football Club and Oldham Rougheyeds Rugby Team, targeted sexual health education and chlamydia screening of the playing staff is being undertaken followed by a mass screening event for fans on match day. Chlamydia information and promotion of screening will be included in match day programmes for four weeks prior to the screening event.Other relevant work includes participation in the forthcoming Men's Health Week in June 2008. This week will have a focus on the workplace and will provide an opportunity to raise awareness, address men's concerns and dispel misconceptions about the chlamydia screening process. There will also be engagement with key media organisations whose target audience is young men.Access to screening for men is monitored quarterly by the Health Protection Agency by measuring at PCT and local programme level the proportion of screens that are taken up by men. These figures are currently fed back quarterly to the programme areas. These figures will be available on the NCSP website later this year.

The National Chlamydia Screening programme (NCSP) published the Men too Strategy in November 2007 in response to the variation observed in the proportion of men screened in the local Chlamydia Screening programmes. All chlamydia screening offices (CSOs) have been made aware of the need to address the issue of equitable access to screening.Discussions on ways to increase uptake amongst men have been undertaken at a number of region-wide chlamydia screening co-ordinator networks. Work in relation to identifying interventions supporting male engagement and the success of each activity is underway through consultation with local Chlamydia Screening programmes. This feedback will be collated to inform a working group which has been convened to advise and steer the implementation of the ““Men too Strategy”” later in 2008.The working group includes representatives from the Men's Health Forum; a men's health promotion specialist from a primary care trust (PCT); a sexual health network director, who commissions and facilitates sexual health service provision; the chief executive of Brook, the young persons sexual health service; and a CSO coordinator.Work in relation to men's perceptions and experiences has been discussed by the working group but no formal study of this has commenced yet.Work on background information and guidance is in the early stages of development. These will be published in the form of a toolkit planned for launch in November 2008.To date, examples of local initiatives include the Greater Manchester strategy to use sport to engage male participation in screening. In partnership with both Oldham Athletic Football Club and Oldham Rougheyeds Rugby Team, targeted sexual health education and chlamydia screening of the playing staff is being undertaken followed by a mass screening event for fans on match day. Chlamydia information and promotion of screening will be included in match day programmes for four weeks prior to the screening event.Other relevant work includes participation in the forthcoming Men's Health Week in June 2008. This week will have a focus on the workplace and will provide an opportunity to raise awareness, address men's concerns and dispel misconceptions about the chlamydia screening process. There will also be engagement with key media organisations whose target audience is young men.Access to screening for men is monitored quarterly by the Health Protection Agency by measuring at PCT and local programme level the proportion of screens that are taken up by men. These figures are currently fed back quarterly to the programme areas. These figures will be available on the NCSP website later this year.

The Department monitors delivery of the objectives of the National Chlamydia Screening Programme (NCSP) through a service level agreement with the Health Protection Agency. The NCSP will monitor progress against the ‘Men too...’ strategy through routine quarterly analysis of disaggregate data, which will be published on the website and in the annual report. Progress on individual elements of the strategy will be monitored by the NCSP programme board.

In National Planning Guidance, primary care trusts (PCTs) have been asked to plan to screen 17 per cent. of their population aged 15-24 for chlamydia in 2008-09.Funding to the national health service for chlamydia screening has been allocated as follows:
From 2006-07, funding to support chlamydia screening was not allocated separately to PCTs but is contained within the mainstream revenue allocations made to PCTs. The 2006-07 and 2007-08 revenue allocations to PCTs separately identified £211 million in 2006-07 and £342 million in 2007-08, around half of the £1 billion promised to support implementation of the ““Choosing Health White Paper””. For the two years covered by this allocation, a total of £70 million was identified nationally to invest in chlamydia screening.From 2008-09 this funding has been included in PCT baselines. It is the responsibility of PCTs to commission the health services they need to meet the health requirements of the local populations they serve.

The National Chlamydia Screening Programme is undertaking research, to explore the relationship between deprivation, screening volumes and chlamydia positivity. The work will be used to develop an evaluation process that can be used to improve equitable access to the screening programme.

The National Chlamydia Screening Programme (NCSP) screens for asymptomatic chlamydia outside of genito-urinary medicine services.Table one shows the number of chlamydia screens among those aged 13-24 years broken down by sex and programme area for England, as part of the NCSP for the period 1 April 2006 to 31March 2007. Table two shows the proportion male and female who were screened for chlamydia by programme area, among those aged 13-24 years as part of the NCSP for the period 1 April 2006 to 31 March 2007.

NICE has not been asked to appraise the enriched culture method (ECM) test for Group B Streptococcus (GBS). NICE is currently revising the approach it takes in considering proposals for appraisals of diagnostics, including the proposal to appraise the ECM test for GBS. NICE’s future approach for considering proposals for appraisals of diagnostics is expected to be considered by NICE’s Board and the Department this year. NICE will consider the suitability of the proposed appraisal of the ECM test for GBS for referral to its work programme following the agreement of a new approach.
