Notes:
Finished Consultant Episode (FCE)
An FCE is defined as a period of admitted patient care under one consultant within one health care provider. Please note that the figures do not represent the number of patients, as a person may have more than one episode of care within the year.
Diagnosis (Primary Diagnosis)
The primary diagnosis is the first of up to 14 (seven prior to 2002-03) diagnosis fields in the Hospital Episode Statistics (HES) data set and provides the main reason why the patient was in hospital.
Secondary Diagnoses
As well as the primary diagnosis, there are up to 13 (six prior to 2002-03) secondary diagnosis fields in Hospital Episode Statistics (HES) that show other diagnoses relevant to the episode of care.
Definition of Selected Diagnoses (ICD-10 Codes)
Type 2 diabetes defined as:
E11 (non-insulin-dependent diabetes mellitus) or O241 (pre-existing diabetes mellitus, non-insulin-dependent) recorded in primary diagnosis or in any of the secondary diagnoses fields.

Tim Loughton: To ask the Secretary of State for Health what steps he is taking to develop a co-ordinated approach to chronic disease management in patients with (a) diabetes, (b) heart disease and (c) kidney disease. [224719] Ms Rosie Winterton: The NHS Improvement Plan set out the Government's priority to improve care for people with long-term conditions by moving from reactive care towards a systematic, patient centred approach. The national service framework (NSF) for long-term conditions was launched on 10 March 2005 and aims to support people with long-term conditions to live as full and independent a life as possible. The Department has set up a vascular programmes board that will oversee delivery of the targets for cardiovascular disease (coronary heart disease, stroke and other circulatory diseases), diabetes, chronic kidney disease, health improvement and prevention, and hypertension. The work of the board will assist in the process of taking a more co-ordinated approach to chronic disease management in patients with diabetes, heart disease and kidney disease. The diabetes NSF was published in December 2001. The NSF sets out a 10-year programme of change where all people with diabetes requiring multi-agency support will receive integrated health and social care. The recently published report, "Improving Diabetes Services--the NSF Two Years On", provides information about how services are changing to provide an integrated approach for people with diabetes. The coronary heart disease (CHD) NSF, published in March 2000, set out a strategy to modernise CHD services over 10 years. Heart failure was identified as an area where there was significant scope to help people with heart failure live longer and achieve a better quality of life. Part two of the NSF for renal services was published in February 2005. It sets out new quality requirements for the identification and early management of people with, or at risk of, chronic kidney disease. It also identifies considerable scope for integrating care pathways for people with chronic kidney disease, coronary heart disease and/or diabetes, to reduce the impact of these interacting long-term conditions.

Tim Loughton: To ask the Secretary of State for Health (1) what assessment he has made of the European Medicines Evaluation Agency's call for more clinical trials on analogues used in diabetes treatment; [210222] (2) what assessment he has made of the possible carcinogenic effect of analogues prescribed for diabetes; [210223] (3 what his policy is on the mitogenic potency of insulin analogues. [210225] Ms Rosie Winterton: Insulins are known to have a weak mitogenic effect on some cells in cell culture. Before any licence is granted, a full and thorough review of all the available data takes place to ensure that the product is of appropriate standards of safety, quality and efficacy. The European regulatory guidelines "Points to Consider", a document on the non-clinical assessment of the carcinogenic potential of insulin analogues provides guidance to industry on the general approach to the assessment of new insulin analogues, and these points need to be addressed before a licence would be granted. For any application, the documentation, including clinical trial details and results, accompanying the formal application for the product licence should be consistent with current European and national legal requirements and guidelines relating to applications for marketing authorisations. As with all new medicines, several insulin analogues are currently under intensive monitoring by the Medicines and Healthcare products Regulatory Agency and the committee on safety of medicines. We are not aware of any clinical evidence to suggest that diabetic patients who receive either human insulins, insulin analogues or animal insulins are at an increased risk of developing cancer. Any further clinical trial data would always be considered to be of benefit and welcomed.
