DFID has recently introduced a new methodology for monitoring AIDS and HIV expenditure. Bilateral expenditure figures based on this methodology are shown in the following table.

The level of detail available on our central systems does not allow us to systematically capture the amount of expenditure on HIV/TB collaborative activities. However, DFID is able to separately identify spending on communicable disease control, of which spending on TB related activities is a part. Table 2 shows estimates of DFID’s bilateral expenditure, excluding poverty reduction budget support (PRBS) on communicable disease control over the same period. This table only includes directly targeted support and excludes other expenditure which will have an impact on TB such as broader support to strengthen health systems.

DFID recognises that the efforts against HIV and TB are inextricably linked. TB incidence rates have at least doubled in Sub Saharan Africa in the last 15 years. 50 per cent. of TB deaths are in people living with HIV and AIDS (PLWHA) in Africa (7 per cent. in south East Asia). TB/HIV co-infection is now the central and most worrying feature of the epidemic. The two diseases must be considered together. The biggest challenge to integration in resource limited settings are human resources. TB diagnosis is also difficult in PLWHA, and TB and HIV drug interactions make treatment very complex. New TB diagnostic tools are needed.DFID is providing support to help countries strengthen their health systems. In Malawi, we are helping with £100 million emergency programme over six years, part of which aims to double the number of nurses and triple and number of doctors, and retain them through better pay and conditions, with a salary increase of 50 per cent. We are also providing support directly to strengthen national TB programmes. For example, in China, DFID has allocated £28 million over seven years towards reducing tuberculosis morbidity and mortality through an effective and sustainable national TB control programme focused on the poor. This has involved raising case detection and treatment through directly observed treatment short course (DOTS) based services.In addition DFID funds a variety of multilateral organisations that contribute to programmes targeting both HIV/AIDS and TB. Most notably we have pledged £359 million to date to support the global fund to fight AIDS, TB and Malaria (GFATM), as our principal support to the provision of TB drugs.

DFID supports country-led national AIDS and tuberculosis (TB) control programmes in a number of high HIV prevalence countries. DFID also made significant contributions to the Global Fund to fight AIDS, TB and Malaria which supports country programmes for AIDS and TB. The UK has committed £359 million to the Fund for 2002-07, including £100 million for 2006 and the same for 2007, subject to performance. We have also made a long-term commitment to UNITAID, the new international drug purchase facility, scaling up to £40 million per year by 2010, subject to performance. These initiatives provide countries with access to increasing resources for scaling up HIV and TB interventions.Most countries with HIV and TB co-epidemics already have national plans to address these epidemics in increasingly co-ordinated programmatic ways and many have also finalised universal access plans to dramatically scale up these responses. Scaling up access to antiretroviral therapy offers opportunities to better control TB and has been shown to decrease the incidence of TB by 70-80 per cent. in people already infected with HIV.DFID is contributing to strengthening national health systems to scale up the delivery of basic services to the poor that include TB and HIV prevention and treatment. An example of this is the Malawi Emergency Human Resources Programme that is making more health workers available to deal with increasing numbers of patients who are infected with both HIV and TB.DFID is also funding research working to identify better ways to deliver services tackling the co-epidemics. We are also supporting the work of WHO, for example the STOP TB programme, which is supporting countries to develop co-ordinated HIV and TB responses.

DFID is very concerned with the spread of the new, highly resistant form of tuberculosis, referred to as extreme drug resistant TB (or XDR-TB). This is virtually untreatable and is made worse by the HIV/AIDS epidemic, because HIV makes people more susceptible to TB.Resistance to anti-TB drugs occurs because of poorly managed TB care. The best way of preventing the development of drug resistance is to ensure all TB patients complete treatment with effective drugs.DOTS (Directly Observed Treatment Short-Course) remains at the heart of the Stop TB Strategy for effective diagnosis and treatment of TB. The World Health Organisation and partner agencies have developed the DOTS-Plus approach for the management of multi-drug-resistant tuberculosis (MDR-TB) which includes rational use of second line drugs.In 2004, DOTS strategy was extended to cover 183 countries, and there were 4.4 million new and relapse TB cases notified, of which 2.1 million were new cases.The UK supports TB control through a variety of channels, including bilateral country programmes and through support to international organisations and partnerships such as the Global Fund to fight AIDS, TB and Malaria (GFATM), the World Bank, the European Commission, WHO and the Stop TB Partnership.DFID is a founder member of UNITAID (the International Drug Purchase Facility) which will be launched this month at the United Nations General Assembly, and will provide additional funding to help the scale-up of services to improve access to treatment for AIDS, TB and malaria.The UK has already pledged to provide £100 million to GFATM in both 2006 and 2007, (subject to performance). We will also provide £5 million to support the work of the Stop TB Partnership and its Global Plan to Stop TB 2006-2015. In January this year, the Chancellor also announced new UK support to combat TB in India, where DFID will provide £41.7 million over five yeas.The development of new drugs will be crucial to the treatment and management of multi-drug resistant (MDR) and XDR-TB. DFID has a long history of supporting research on TB and we currently support research programme consortia on communicable disease at the London School of Hygiene and Tropical Medicine and the Nuffield Centre for International Health at Leeds University. Both programmes will receive £5 million each over the next five years.DFID is also supporting the research and development of TB drugs and diagnostics via WHO’s programme on Tropical Disease Research (TDR) and the Global Alliance for TB Drugs. The Global Alliance will receive £6.5 million from 2005-08 for the development of new TB drugs.
