"Clinical Practice Guidance for the Assessment of Young Women aged 20-24 with Abnormal Vaginal Bleeding" was issued through the Department of Health's GP and Practice Team Bulletin and Chief Nursing Officer's Bulletin in March 2010. The guidance, developed by the independent Advisory Committee on Cervical Screening (ACCS), re-emphasises the importance of pelvic examinations in young women with abnormal vaginal bleeding and provides primary care practitioners with an easy to follow algorithm to help in their management of these young women. A copy of the guidance has been placed in the Library.Regarding research on the detection and diagnosis of cervical cancer in general and among those aged between 20 and 24 years in the last 12 months, NHS Cancer Screening Programmes provided funding of £52,000 in 2009-10 to Cancer Research UK on the ongoing audit of invasive cervical cancers. This included funding for specially commissioned work on younger women to inform the ACCS review of cervical screening in May 2009.In addition, in autumn 2009 the Department commissioned the "What Women Know" survey to assess the current levels of knowledge about cervical cancer among women in England. The survey was carried out as part of an initiative in collaboration with the Eve Appeal and University College London's Health Behaviour Research Centre. It covered awareness of the symptoms and risk factors for cervical cancer, knowledge of the cervical screening and human papillomavirus vaccination programmes, anticipated barriers to seeking medical help with a possible cervical cancer symptom, and beliefs about screening and other protective behaviours.The Department has also provided £4,000 to Jo's Trust, a charity that raises awareness of cervical cancer, to undertake a pilot to assess the potential for using television advertisements in general practitioner surgeries to improve uptake of cervical screening invitations.

The NHS Health Check programme is a universal and systematic programme for everyone between the ages of 40 and 74 that will assess individuals' risk of heart disease, stroke, kidney disease and diabetes and will support people to reduce or manage that risk through individually tailored advice. The programme was developed on the basis of advice from the National Screening Committee, is based on guidance produced by the National Institute of Health and Clinical Excellence (NICE) and is both cost effective and clinically effective.The risk assessment stage of the NHS Health Check uses a risk engine to calculate a person's 10-year risk of cardiovascular disease. Evidence on cardiovascular risk estimation has developed considerably, with a number of risk engines available, including Framingham and QRISK. These have been calibrated to reflect risk factors within specific populations, such as ethnicity, and a number of scientific reviews have been published on their validity. The Department takes its advice from NICE on matters such as which risk engines are most appropriate for the national health service to use. NICE has recently revised its guidance to allow the NHS to decide locally which risk engine best predicts risk for their population mix.Of equal importance however is that people are given individually tailored advice and the necessary follow up which is suitable for their particular circumstances to help them manage or reduce their risk. The advice, interventions and follow up that are offered will involve professional judgment and will help ensure that everyone's individual needs are met.

For period 2008-09 (the latest period for which the figures are available) 78.6 per cent. of women did not receive their cervical screening test results within two weeks. However, test results were available for 21.4 per cent. of women within two weeks compared to 11.1 per cent. in 2007-08, and 65.5 per cent. were available within four weeks compared to 59.6 per cent. in 2007-08.It is a Vital Sign of the 'NHS Operating Framework 2010-11', a copy of which has already been placed in the Library, that all women should receive the results of their cervical screening tests within two weeks by 2010. 10 pilot programmes have achieved 14-day turnaround times for at least 99 per cent. of tests and with significant reductions in failed tests and in resource requirements. This is now being rolled out across the country.

Immediate implementation was considered as an option and a full analysis of the cost and mortality rates of this approach was made and compared to the analysis of phased implementation. However in common with all other national screening programmes, this programme needs to have a managed national roll out to realise the benefits of the combination of screening to safe and effective standards. Early implementation would not have allowed sufficient time to reorganise treatment services in order to minimise mortality and morbidity in patients having planned surgery.

This is made up of start up and full year costs for six early implementation sites, start up costs for four phase one sites, screening equipment for 10 sites, training of technicians and information leaflets.It is anticipated that there will be a maximum of eight phases of roll out, however the exact number is unknown as this depends on the number of applications received and geographical fit of each local screening programme.The following national health service trusts have been successful with their applications to begin ultrasound screening for aortic aneurysms:
It is anticipated that full national roll out of the Abdominal Aortic Aneurysm screening programme will be completed by the end of 2012-13.The following table details the numbers of patients screened in phase one after a) oneyear and b) five years:
All phase one sites will be screening by the end of May 2010.The national programme team have put in place a number of measures to ensure the highest possible take up of screening once men have been identified for screening through their general practitioner. Examples include:"Involving 65 year old men when developing the initial communication materials for the screening programme to ensure that key messages were targeted at the right level;""National information leaflets have been designed for use in local programmes to ensure consistency of messages and offer informed choice;""Recruiting a Communications manager to work with local programmes ensuring that the right information is promoted to suit their local demographic; and""Commissioning an independent evaluation of all information to assess:""whether the patient information materials met the needs of men invited for screening""how well men invited for screening understand the risks they may face and;""assess how well health professionals think the information meets the needs of men invited for screening and the programme itself."

The NHS Abdominal Aortic Aneurysm screening programme has cost £5,335,809.50 up to the end of March 2010. This is made up of start up and full year costs for six early implementation sites, start up costs for four phase one sites, screening equipment for 10 sites, training of technicians and information leaflets.It is anticipated that there will be a maximum of eight phases of roll out, however the exact number is unknown as this depends on the number of applications received and geographical fit of each local screening programme.The following national health service trusts have been successful with their applications to begin ultrasound screening for aortic aneurysms:

Primary care trusts (PCTs) are responsible for screening services, including the promotion of the national health service bowel screening programme. We would expect those areas with low bowel screening uptake rates to develop targeted programmes to tackle this locally.As the bowel cancer screening programme was implemented in local areas, NHS Cancer Screening Programmes sent out an information pack to all local general practitioners (GPs). As well as explaining how the programme works, the packs contained promotional information, including posters for GPs to use to promote the programme in their own practices.The National Awareness and Early Diagnosis Initiative, launched in November 2008, is working with NHS Cancer Screening Programmes on key messages and awareness programmes to improve screening uptake, particularly in deprived groups.

The NHS Health Check programme is a Tier 3 Vital Sign in the NHS Operating Framework 2010-11. Consequently, strategic health authorities play a significant role in monitoring the roll-out of the programme locally. To support national understanding of the pace of implementation of the programme, data will be collected by the Department from 1 April 2010 on the number of people between the ages of 40 and 74 who have received an NHS Health Check. An NHS Health Check data set is also being developed which will enable the national health service to observe the progress of the programme in more detail.Funding for the programme is not ring-fenced and is provided as part of general primary care trust (PCT) allocations. Data on spend by PCTs on implementing the programme are not collected centrally.
