Mr. Burstow: To ask the Secretary of State for Health whether his Department monitors hospital and community clinicians' attendance at infection control training; and if he will make a statement. [138190] Mr. Hutton: For nurses, midwives and other non- medical health professionals, it is for local employers to determine their training needs linked to local service plans. Access to training is affected by whether funding is available, staff can be released, training interventions are appropriate, flexible and convenient and mentors/ assessors are available. It would not be practical for the centre to be prescriptive on this. Acute trusts have to provide education on infection control as part of the controls assurance standard. Recently published guidance from the National Institute for Clinical Excellence on infection control in primary and community care includes recommendations on training. For doctors, the content and standard of postgraduate medical training is the responsibility of the United Kingdom competent authorities, the specialist training authority for specialist medicine and, for general practice, the joint committee on postgraduate training for general practice, which will both be replaced by the postgraduate medical education and training board. Their role is that of custodians of quality standards in postgraduate medical education and practice. They are independent of the Department. In addition, the General Medical Council's education committee has the general function of promoting high standards of medical education and co-ordinating all stages of medical education to ensure that students and newly qualified doctors are equipped with the knowledge, skills and attitudes essential for professional practice. All of these bodies have a vested interest in ensuring that doctors are equipped to deal with the issues they will encounter in practice--including where appropriate, those relating to infection control. However, it is neither practicable nor desirable for the Government to prescribe and monitor the exact training that any individual doctor will receive.

Mr. Burstow: To ask the Secretary of State for Health what the backlog maintenance cost was to reach (a) estate code condition B, (b) fire safety standards and (c) health and safety standards for all mental health trusts in England, broken down by trust. [133847] Mr. Hutton: The information requested is set out in the table. The figures provided do not indicate levels of risk to patients and staff. Trusts make every effort to remedy significant concerns as quickly as possible. This is reflected in the very small numbers of injuries and deaths arising as a result of fires and reportable health and safety incidents in national health service hospitals in England, despite the very high numbers of people who use healthcare premises. Spending on routine maintenance by mental health trusts reached £41 million in 2002-03 with an additional £60 million having been spent between 1998 and 2001. Mr. Burstow: To ask the Secretary of State for Health pursuant to his Answer to the honourable Member for Truro and St Austell of 6th February 2003, Official Report, column 450W, on performance targets, how his Department will measure progress towards achieving the public service agreement target on value for money. [138199] Mr. Hutton: The value for money Public Service Agreement (PSA) target requires that `Value for money in the National Health Service and personal social services will improve by at least 2 per cent. per annum, with annual improvements of 1 per cent, in both cost efficiency and service effectiveness'. For the NHS, the cost efficiency element of the PSA target will be measured by comparing data from the National Schedule of Reference Costs between one year and the previous year. Adjustments to this simple comparison will be made to take account of changes in casemix, quality investment, input price inflation and externally driven, one-off, cost-increasing events such as rising employer pensions contributions. The quality element of the target will be assessed by comparing the estimated expenditure on increasing service effectiveness with data on quality from the NHS performance ratings exercise. A key element of this will be the value that can be placed on the lives saved from reducing mortality following health care interventions. For personal social services, costs will be based on unit costs of services, after some adjustments to allow for increases in funding for quality improvements. The quality element will be based on the quality indicators published by the Department, in the social services performance assessment framework, for instance, waiting times for care packages.

Mr. Burstow: To ask the Secretary of State for Health how the Government will ensure that the National Service Framework for Long Term Conditions is given sufficient priority by primary care trusts; and what the reasons were for the decision not to set milestones and targets. [138461] Dr. Ladyman: Our strategy to support delivery of the NHS Plan is to set a clear national framework of standards backed up by strong inspection regimes and increasingly to give more responsibility to front-line health and social services providers so that local organisations have more flexibility and freedom to plan, resource and deliver services. As a result we set fewer central targets. Progress in policy areas not directly linked to national targets can be monitored and driven forward in a number of ways, for example: Primary care trusts can set local targets and performance manage progress locally. The Commission for Health Improvement (to become, subject to legislation, the Commission for Healthcare, Audit and Inspection) and the Social Services Inspectorate (to become, again subject to legislation, the Commission for Social Care Inspection) will take account of nationally set standards including national service frameworks when inspecting hospitals and other providers, and may undertake themed inspections. Patients' forums will be able to identify where services are not meeting the standards. Patient feedback, both informal and formal (for example, patient surveys), will provide a local means of identifying shortfalls in services. Policies like the expert patients programme and the patients choice programme are also giving people an increasing say in local services and their own treatment.

The increase in July's turnaround time figure can be attributed to a surge in applications during the last two weeks in June in advance of the increase in fees taking effect. The Criminal Records Bureau received approximately 50,000 more applications than its usual intake during this period. While this increase in applications temporarily increased the turnaround times for applications, the Criminal Records Bureau has continued to meet its standard of service targets.
